Application for an extract from the register in electronic form

The form is used to request an extract from the register of selected industrial law. The extract is provided in electronic form and has an electronic signature.

To process a request:

1. Completing an electronic request for an extract

2. Sending electronic information about the amount and method of payment to the specified e-mail address of the applicant

3. After payment of the administrative fee, send the statement to the specified e-mail address of the applicant
